Esta tesis doctoral versa sobre el análisis de la mortalidad por lesiones de
tráfico en usuarios de la vía en España, un tema de importancia en Salud
Pública por su magnitud y transcendencia sociosanitaria y que no está su-
ficientemente estudiado en nuestro país. El objetivo general de la tesis ha
sido analizar las diferencias en las tasas de mortalidad por lesiones de tráfico en peatones y conductores cuantificando la contribución relativa de cada
componente de la mortalidad (exposición, accidentalidad y letalidad) en
función del año (evolución temporal), y de la edad y el sexo. Para ello, se
emplearon distintas fuentes de datos (Registro de Accidentes de Tráfico con
Víctimas y Registro Español de Licencias de Conductores de la Dirección
General de Tráfico de España; y Censo de Población del Instituto Nacional
de Estadística). El enfoque de análisis se basó principalmente en el método
de descomposición, una herramienta validada para descomponer la tasa de
mortalidad por lesiones de tráfico en sus tres componentes (exposición, accidentalidad y letalidad), cuantificar el peso de cada componente sobre los
cambios en la tasa de mortalidad y comparar grupos estratificando según
factores de riesgo (como la edad o el sexo). Además, se utilizó el método
de exposición cuasi-inducida para obtener estimaciones de la exposición en
peatones y conductores, y modelos de regresión de Poisson para la obtención
de estimaciones puntuales. Del análisis realizado pueden destacarse varios
resultados. El componente más determinante en la disminución de la tasa de
mortalidad por lesiones de tráfico en peatones fue la letalidad y, en menor
medida, la accidentalidad (o riesgo de colisión entre un peatón expuesto y
un vehículo). Del mismo modo, en conductores, el factor más determinante
en la disminución de la tasa de mortalidad por lesiones de tráfico fue la
letalidad, mientras que la exposición también contribuyó aunque en menor
medida. Como era de esperar, la tasa de mortalidad por lesiones de tráfico en
peatones aumentó con la edad; en cambio, en conductores, _esta presentó un
patrón en forma de U detectándose el exceso de mortalidad en el grupo mas
joven (de 15 a 17 años), si bien este patrón se modificó tras ajustar por la
exposición cuando el mayor exceso de mortalidad se detectó a partir de los 75
años. En ambos tipos de usuarios, el componente que más contribuyó a este
exceso de mortalidad relacionado con la edad fue la letalidad. La exposición
aumentó con la edad en peatones y descendió en conductores, y la accidentalidad
(o riesgo de colisión) disminuyó con la edad en peatones y aumentó
en conductores. Los varones, tanto peatones como conductores, tenían tasas
de mortalidad más altas en comparación con las mujeres. En peatones, la
letalidad fue el principal determinante de esta diferencia. Sin embargo, su
mayor letalidad, y también su mayor riesgo de involucrarse en una colisión,
se contrarrestó parcialmente por su menor cantidad de exposición, observada
en los peatones del sexo masculino de todas las edades, excepto en edades
extremas (de 0 a 14, y a partir de los 75 años). En cambio, en los conductores
varones las diferencias se explicaban por la mayor exposición (especialmente
en el grupo de 45 a 74 años) y letalidad (especialmente en el grupo de 15 a 44
años, y a partir de los 75 años). En conclusión, la magnitud y dirección de las
asociaciones entre la edad, el sexo y el año, y cada uno de los tres componentes
de la tasa de mortalidad por tráfico en peatones y conductores diferían
según el componente analizado. Estas diferencias deben tenerse en cuenta
para priorizar estrategias preventivas destinadas a disminuir la mortalidad
debida a lesiones por tráfico en los distintos usuarios de la vía.
