El objetivo principal de esta tesis doctoral
ha sido el de estudiar el impacto de ciertas alteraciones genéticas en los receptores de
los estrógenos y la progesterona y genes implicados en el transporte y metabolismo
de las hormonas esteroideas, así como en genes de vías de señalización implicadas en
la regulación de la respuesta inmunitaria. Al mismo tiempo, evaluamos si los
marcadores genéticos seleccionados en estas rutas y aquellos identificados por GWAS
podrían jugar un papel destacado en la regulación de la respuesta frente a fármacos
anti-TNFα y la aparición de erosiones óseas. Considerando las diferencias de incidencia
entre sexos y el papel atribuido al factor reumatoide o los anticuerpos antipeptídicos
citrulinados (FR yanti-CCP) en la modulación de la respuesta terapéutica y la progresión
de la enfermedad, decidimos además evaluar si el efecto de los marcadores genéticos
seleccionados podía verse afectado por estos factores. Finalmente, con el objetivo de
trasladar nuestros hallazgos a la práctica clínica elaboramos modelos predictivos con
el fin de predecir con mayor certeza la respuesta terapéutica a los diversos fármacos
anti-TNF que se usan en la práctica clínica. Todos los estudios genéticos se
desarrollaron en el contexto del consorcio internacional REpAIR (RhEumatoid Arthritis
International Research consortium) que nuestro grupo coordina.
