En la presente tesis, hemos realizado la evaluación biológica de diversos agentes
teragnósticos específicos para el diagnóstico y el tratamiento de tumores que
actualmente presentan un mal pronóstico y problemas de resistencias y recurrencias
como son el cáncer de mama triple negativo, el osteosarcoma o el cáncer de cérvix
uterino. (10–12)
Para ello, se han ensayado los agentes teragnósticos PEI-NIR-780, que permiten
simultáneamente el diagnóstico específico gracias a la sonda NIR-780 que fluoresce en
el infrarrojo cercano (NIR, near infrared) y la terapia génica gracias a la polietilenimina
(PEI) que permite establecer fuerzas electrostáticas con el material genético a través de
sus aminas secundarias y terciarias. (13,14) La fluorescencia en el infrarrojo cercano
permite el diagnóstico del cáncer en estadíos tempranos debido a su alta sensibilidad,
permitiendo la posibilidad de diagnóstico in vivo y evitando la autofluorescencia de los
tejidos. Las sondas NIR-780 presentan además la capacidad de transporte dirigido
específicamente hacia las células tumorales. (15,16) En la presente tesis, hemos
demostrado que estas nanopartículas tienen una buena capacidad de unión al DNA,
protección de la degradación de las DNAsas, una baja citotoxicidad y una buena
eficiencia de transfección.
Además, permiten su funcionalización con b-ciclodextrinas para la inclusión de
agentes quimioterapéuticos poco solubles como es la doxorubicina para aumentar su
solubilidad, permitiendo su liberación controlada y favoreciendo su transporte dirigido
hacia las células tumorales. (17,18) Asimismo, las aminas secundarias y terciarias de PEI
han permitido su funcionalización con moléculas directoras como bifosfonatos o ácido
hialurónico para su transporte dirigido y específico hacia sus dianas moleculares sobreexpresadas
en los tejidos tumorales.
Las nanopartículas de PEI recubiertas con bifosfonatos (PEI-BP) han demostrado
su capacidad de transporte dirigido específico de la doxorubicina hacia los
osteosarcomas y las metástasis óseas causadas por el cáncer de mama triple negativo,
debido a su afinidad por el hidroxiapatito, el principal constituyente del tejido óseo, y
de los agentes de diagnóstico como es la indocianina green en los modelos animales con
xenografts de ambos tipos de cáncer. Además, estas nanopartículas pueden dirigir el
agente terapéutico hacia las mitocondrias, como un mecanismo alternativo de
citotoxicidad en las células tumorales resistentes al tratamiento. (19)
Por otra parte, las nanopartículas PEI-NIR-780 recubiertas con el ácido
hialurónico (HA) de 40-50 kDa han demostrado la capacidad de diagnóstico específico y
transporte dirigido de la doxorubicina y del material genético tanto en los cultivos
celulares como en los modelos animales de cáncer de mama triple negativo, que sobreexpresa
los receptores CD44 específicos para HA. (20) Asimismo, hemos demostrado
que estas nanopartículas son versátiles y permiten actuar como agentes teragnósticos
también en otros tipos de cáncer CD44+, como es el cáncer cervical y, además, son
capaces de evitar la cardiotoxicidad asociada al uso de la doxorubicina.
Por tanto, hemos demostrado que nuestros agentes teragnósticos son
nanosistemas multifuncionales que permiten el tratamiento y el diagnóstico específicos
y selectivos del cáncer, al dirigir específicamente y de forma eficiente el agente
terapéutico (material genético y/o fármaco quimioterapéutico) al tejido tumoral sin
afectar los tejidos sanos y permitiendo una monitorización del proceso en tiempo real.
Además, a pesar de que existen diferentes sub-tipos de cáncer de mama, en la
práctica clínica se administra un tratamiento estándar en todas las pacientes (21) y se
ha demostrado que las interacciones de las células tumorales con el microambiente
tumoral interfieren en la eficacia terapéutica y en las resistencias al tratamiento. (22)
Por tanto, hemos realizado un cribado de siete fármacos quimioterapéuticos
comúnmente utilizados en la terapia clínica en cinco sub-tipos de células de cáncer de
mama cultivadas en el modelo tridimensional de plasma humano (HuP3D) y hemos
demostrado que el microambiente tumoral interfiere en la respuesta terapéutica
originando una gran heterogeneidad y la necesidad de utilizar distintos parámetros para
la evaluación de la eficacia terapéutica de los fármacos. Asimismo, se ha observado que
las nanopartículas PEI-NIR-780 recubiertas con el ácido hialurónico permiten un
transporte dirigido y diagnóstico específicos en las células de cáncer de mama triple
negativo cultivadas en HuP3D.
Por último, hemos ensayado nuevos agentes de diagnóstico basados en las
sondas fluorescentes NIR-783 y en las nanopartículas fluorescentes Carbon Dots
conjugadas con los carbohidratos manosa, lactosa, N-acetilglucosamina y glucosa y
hemos demostrado su baja citotoxicidad y su especificidad hacia las células tumorales
que sobre-expresan los receptores GLUT1, galectina-3 y asialoglicoproteínas. In this thesis, we have carried out the biological evaluation of various specific
theragnostic reagents for the diagnosis and treatment of tumors that currently have a
poor prognosis and high resistance and recurrence levels, such as triple-negative breast
cancer, osteosarcoma, and cervical cancer. (10–12)
For this purpose, the PEI-NIR-780 theragnostic agents have been tested, which
simultaneously allow specific diagnosis thanks to the NIR-780 probe that fluoresces in
the near-infrared spectrum and gene therapy thanks to polyethyleneimine (PEI) that can
establish electrostatic forces with genetic material through its secondary and tertiary
amines. (13,14) The near-infrared fluorescence allows the cancer diagnosis in the early
stages due to its high sensitivity, allowing the possibility of in vivo diagnosis and avoiding
the tissue autofluorescence. Also, the NIR-780 probe can target specifically to tumor
cells. (15,16) In the present thesis, we have shown that these nanoparticles have good
DNA binding capacity, protection from DNAse degradation, low cytotoxicity, and good
transfection efficiency.
Also, they allow its functionalization with b-cyclodextrins for the inclusion of
poorly soluble chemotherapeutic drugs such as doxorubicin to increase its solubility,
allowing its controlled release and targeting towards tumor cells. (17,18) Likewise, the
secondary and tertiary amines of PEI have allowed their functionalization with
bioligands such as bisphosphonates or hyaluronic acid for their targeted and specific
transport to their molecular targets overexpressed in tumor tissues.
The nanoparticles coated with bisphosphonates (PEI-BP) have demonstrated
specific targeting of doxorubicin towards osteosarcomas and bone metastases caused
by triple-negative breast cancer, due to their affinity for hydroxyapatite, the main
constituent of bone tissue, and specific transport of diagnostic agents such as
indocyanine green in animal models with xenografts of both cancer types. Besides, these
nanoparticles can direct the therapeutic agents towards the mitochondria, as an
alternative mechanism of cytotoxicity in tumor cells resistant to treatment. (19)
On the other hand, PEI-NIR-780 nanoparticles coated with hyaluronic acid (HA)
of 40-50 kDa have demonstrated the capacity for specific diagnosis and targeted
transport of doxorubicin and genetic material in cell cultures and animal models of triple-negative breast cancer, over-expressing HA-specific CD44 receptors. (20)
Furthermore, we have shown that these nanoparticles are versatile and can act as
theragnostic reagents also in other types of CD44 + cancer, such as cervical cancer, and,
also, they can avoid the cardiotoxicity associated with the use of doxorubicin.
Therefore, we have shown that our theragnostic agents are multifunctional
nanosystems that allow the specific and selective cancer treatment and diagnosis by
specifically and efficiently targeting the therapeutic agent (genetic material and/or
chemotherapeutic drug) to tumor tissue without affecting healthy cells and allow realtime
monitorization.
In addition, a standard clinical treatment is used in all breast cancer patients even
though different molecular sub-types can be resistant to the treatment. (21) Also, it has
been shown that the interactions of tumor cells with the tumor microenvironment
interfere with the therapeutic efficacy and can cause drug resistance. (22) Therefore,
we have screened seven chemotherapeutic drugs commonly used in clinical therapy in
five breast cancer cells sub-types cultured in the three-dimensional model of human
plasma (HuP3D) and we have shown that the tumor microenvironment interferes in the
therapeutic response, causing great heterogeneity and the need to use different
parameters for the evaluation of the drug efficacy. Likewise, it has been observed that
PEI-NIR-780 nanoparticles coated with hyaluronic acid allow specific targeting and
diagnosis in triple-negative breast cancer cells cultured in HuP3D.
Finally, we have tested new diagnostic agents based on the fluorescent NIR-783
probes and the fluorescent Carbon Dots nanoparticles conjugated with the
carbohydrates such as mannose, lactose, glucose, and N-acetylglucosamine and we
have demonstrated their low cytotoxicity and their specificity towards tumor cells that
overexpress the GLUT-1, galectin-3, and asialoglycoproteins receptors.
