Alteraciones musculoesqueléticas y deglutorias en supervivientes de cáncer de cabeza y cuello. Eficacia de un programa de terapia manual para el tratamiento de sus secuelas

Abstract

Los resultados de esta Tesis Doctoral Internacional evidencian 1) la presencia de un mayor número de puntos gatillo miofasciales en los sCCyC y mayor hipersensibilidad en las regiones facial, cervical y del hombro en comparación con los valores presentados en personas sanas; 2) la disminución de las funciones motora (a nivel de la apertura de la boca y del rango de movimiento articular cervical y del hombro) y deglutoria así como la interacción entre ambas funciones; 3) la influencia de factores demográficos (edad, género, consumo de alcohol) y clínicos (región y estadio tumorales, tipo de tratamiento médico curativo recibido, función motora y sensitiva de la lengua) sobre la función deglutoria en los sCCyC a largo plazo; 4) la eficacia de un programa de terapia manual – inducción miofascial sobre la función motora en términos de apertura de la boca y rango de movimiento articular cervical y del hombro. Es, por tanto, fundamental la adecuada evaluación de las secuelas presentes tras el tratamiento médico curativo, analizar la asociación entre las mismas y destacar los factores que influyen en su evolución, de forma que pueda alcanzarse un adecuado abordaje y recuperación de las mismas.

The results of this International Doctoral Thesis show 1) the presence of a greater number of myofascial trigger points in the sHNC and greater hypersensitivity in the facial, cervical and shoulder regions compared with the values presented in healthy populations; 2) the decrease in motor (in terms of mouth opening and cervical and shoulder range of motion) and swallowing functions as well as the interaction in within; 3) the influence of demographic (age, gender, alcohol consumption) and clinical factors (region and tumor stage, type of curative treatment performed, motor and sensory function of the tongue) on the swallowing function in the sHNC in the short and long term; 4) the efficacy of a manual therapy program - myofascial induction on motor function in terms of mouth opening, cervical range of motion and shoulder range of motion. It is therefore essential to properly evaluate the side effects present after curative treatment, to analyze the association between them and to highlight which factors are influencing their evolution, so that an adequate rehabilitation approach and an optimal recovery can be achieved.
