Probablemente el mayor desafío a la hora de abordar como
neurocirujano el problema de la hemorragia intracerebral espontánea (HIE) sea
el de la incertidumbre sobre la indicación quirúrgica. Sin menospreciar la
pericia técnica necesaria para la evacuación de un hematoma espontáneo
(sobre todo cuando subyace bajo él un aneurisma roto o una malformación
arteriovenosa), podría decirse que una de las mayores dificultades para el
cirujano en esta patología consiste en decidir qué pacientes deben operarse y
cuándo.
El papel de la cirugía y sus indicaciones están mayormente
consensuados en las HIE infratentoriales, donde mejor definidos están los
criterios quirúrgicos y el momento en que debe de realizarse la intervención, sin
embargo las indicaciones de la cirugía para la HIE supratentorial varían
ampliamente en todo el mundo, lo que refleja la incertidumbre sobre sus
efectos.
Esta incertidumbre en la práctica clínica habitual y la existencia de metaanálisis
que apuntaban hacia una mejoría con el tratamiento quirúrgico, nos
hizo plantear un estudio en el que analizaremos el efecto de la cirugía en los
pacientes con HIE supratentorial.
