La insuficiencia tricuspídea (IT) es una enfermedad que se caracteriza por la
imposibilidad del correcto cierre de la válvula tricúspide por alteración primaria
de los distintos componentes de la estructura valvular o la disfunción valvular
secundaria a patología pulmonar, las válvulas cardíacas izquierdas (sobre
todo la válvula mitral) y/o las cámaras cardíacas. En la mayoría de estos
pacientes el principal problema de la válvula tricúspide no es primario, sino
secundario a una valvulopatía izquierda concomitante.
La prevalencia en la población es del 15%1 aumentando considerablemente
en pacientes con enfermedades que afectan al corazón izquierdo. El 35% de
los pacientes con insuficiencia cardíaca tiene IT de un grado superior a 2 sobre
4, el 30% de los pacientes con insuficiencia mitral severa y el 50% de los
pacientes intervenidos quirúrgicamente de la válvula mitral2. La IT es por tanto
la complicación más frecuente de la enfermedad valvular mitral. La etiología de
la enfermedad mitral condiciona la prevalencia de la afectación: el 35% de los
pacientes con valvulopatía mitral reumática tienen afectación tricuspídea, el
30% con regurgitación mitral isquémica y el 15% con prolapso valvular mitral3.
La IT y su grado son predictores independientes de mortalidad tras ajustarlo
por el resto de variables: edad, fracción de eyección de ventrículo izquierdo,
presión sistólica de la arteria pulmonar, tamaño o función de ventrículo derecho
(VD) y dilatación de la vena cava inferior. La mortalidad se incrementa al
aumentar su severidad y la persistencia de un grado 3-4/4 tras el recambio
valvular mitral se asocia en el seguimiento a peor grado funcional de la New
York Heart Association (NYHA), a mayor mortalidad por insuficiencia cardíaca y
a mayor mortalidad por todas las causas3.
El incremento de la morbimortalidad es clínicamente muy relevante. En el
seguimiento a 10 años más de la mitad de los pacientes con IT de grado
avanzado desarrollarán clínica de insuficiencia cardíaca en estadio funcional IIIIV/
IV de la NYHA, describiéndose supervivencias del 65% en los pacientes con
insuficiencia valvular tricuspídea severa frente a un 90% en aquellos sin
insuficiencia.
Clásicamente pensábamos que con el tratamiento quirúrgico de la valvulopatía izquierda mejoraría la valvulopatía tricuspídea y por tanto no había
necesidad de intervención quirúrgica sobre la misma. Sin embargo, a lo largo
de la última década se ha demostrado que puede no mejorar e incluso
empeorar a pesar del tratamiento de la valvulopatía izquierda. Como ejemplo,
sólo el 33% de la IT severa/moderada tras valvuloplastia mitral pasa a ser
ligera4. A pesar de las importantes implicaciones clínicas y pronósticas, es muy
limitada la evidencia acerca de cuál es el manejo diagnóstico preoperatorio
adecuado y la actitud quirúrgica sobre esta válvula de forma aislada o asociada
a otra cirugía cardíaca.
El estudio de Dreyfus y cols5 fue uno de los más representativos en este
campo al mostrar que el tratamiento de la lesión izquierda primaria aislada no
corregía la dilatación del anillo tricuspídeo ni mejoraba la función del ventrículo
derecho, que son los principales determinantes en la progresión de la
insuficiencia en el seguimiento. Éste y otros grupos quirúrgicos han intentado
determinar los factores preoperatorios asociados a la progresión postoperatoria
de la regurgitación y han fomentado por ello un tratamiento activo de la lesión
tricuspídea. Entre los factores encontrados asociados a la progresión de la IT
postoperatoria cabe destacar: características demográficas, factores
mecánicos, remodelado de cavidades, fibrilación auricular y enfermedad
orgánica oculta de la válvula tricúspide. Por otra parte, algunos de los factores
considerados clásicamente muy determinantes como la hipertensión pulmonar
no han sido confirmados en algunos estudios6. En general, se considera que la
relación es compleja y probablemente dependiente de un conjunto de
parámetros y de su interacción, algunos de ellos no totalmente establecidos en
el momento actual.
En los últimos años se han añadido otros posibles factores como el implante de
dispositivos (DAI y marcapasos), mostrando que un porcentaje de pacientes
con IT ligera previo al implante desarrollan IT de grado avanzado en el
seguimiento. Los mecanismos pueden ser debidos a que la posición de los
electrodos interfiera con el movimiento de alguno de los velos de la tricúspide,
a adherencias y fibrosis del cable, o a perforación de velos.
Otro tema controvertido es el tipo de intervención quirúrgica a realizar. La
técnica de elección en su tratamiento es la reparación valvular. El abordaje más
simple y extendido es el tratamiento exclusivo de la dilatación del anillo, que
puede realizarse mediante anuloplastia con anillo protésico preferiblemente rígido y que en la mayoría de las series ha objetivado beneficios respecto a
técnicas de sutura, habiendo demostrado una regurgitación residual con anillo
del 15% y sin anillo del 20-35%7. Cuando existe una deformidad muy
significativa de la válvula tricúspide o un remodelado ventricular derecho
adverso, la anuloplastia es insuficiente, asociándose a regurgitación residual y
a la progresión de la misma tras la cirugía. Por tanto, se recomienda añadir
otras técnicas reparadoras como la ampliación del velo anterior. En escasas
ocasiones la válvula presenta otras alteraciones anatómicas (prolapso,
perforación, etc) que son susceptibles de otras técnicas quirúrgicas específicas
(implante de neocuerdas, decalcificación, comisuroplastia, etc). El reemplazo
valvular por válvula biológica o mecánica es la técnica de última elección. Sin
embargo, no están establecidos los parámetros anatómicos de la válvula o del
ventrículo que aconsejarían una u otra técnica reparadora o la imposibilidad de
ninguna de ellas.
Hablamos por tanto de una enfermedad de larga evolución con una importante
repercusión y relevancia clínica que condiciona la clase funcional, la capacidad
de ejercicio y la supervivencia de quienes la padecen.
