El objetivo de esta revisión es revisar el uso de ventilación mecánica no invasiva (VMNI) en la insuficiencia
respiratoria aguda (IRA) en el servicio de urgencias.
Para ello se realizará una revisión sistemática cualitativa en distintas bases de datos, identificando
artículos con la temática expuesta con una estrategia única; limitado a hallazgos de los últimos 5 años
en español e inglés.
En un estudio realizado en la población pediátrica se concluyó que el manejo precoz de la IRA y el
uso de cánulas nasales de alto flujo reducía el tiempo de hospitalización, las derivaciones a hospitales
especializados, y otorgaba más autonomía a los hospitales regionales en su manejo, disminuyendo
así el número de pacientes que precisaban del uso de técnicas invasivas. En el caso de pacientes con
reagudización de la enfermedad pulmonar obstructiva crónica (EPOC), y su manejo precoz en el ámbito
prehospitalario, se observó un aumento de la mortalidad en aquellos pacientes que fueron sometidos
a alto flujo comparado con la oxigenoterapia convencional. Según los resultados de un metanálisis,
se revisó que no había ningún beneficio en el uso de cánulas nasales de alto flujo frente a la terapia
convencional o la VMNI, en el servicio de urgencias, en términos de necesidad de intubación, fallo de
tratamiento, hospitalización y mortalidad.
Los datos no son concluyentes en ninguno de los estudios revisados y no hay acuerdo entre autores.
Existe escasa bibliografía respecto al uso de VMNI en el servicio de urgencias, debido a que la gran
mayoría de los estudios se centran en el uso de esta técnica en las unidades de cuidados intensivos.
En conclusión existe heterogeneidad en los resultados de los artículos revisados respecto al uso de
VMNI en el servicio de urgencias.; The aim of this study si to revise the use of non-invasive ventilation (NIV) in patients with acute
respiratory failure in emergency departments.
A systematized database review will be carried out by the search of articles attending the presented
subject following a unique approach; restricting the results to findings in the last five years
either in Spanish or English. A research performed in paediatric population concluded that the
early management of acute respiratory failure and the use of high flow nasal cannulae reduced
the hospitalization period and the referral to specialized hospitals, and gave more independence
to regional hospitals in its management; reducing, therefore, the number of patients needing the

Several pathologies may affect the histological structure and physiology of the human oral mucosa,
being necessary to use oral mucosa grafts. In this context, the scarce number of donors has increased
the need to develop artificial substitutes of human oral mucosa, most of them, without attending to
the histological differences found in the different anatomical areas of the oral cavity. The objective
of the present bibliographic review is to carry out a histological description and a morphological
classification of the human oral mucosa and to determine the translational potentiality of the new
artificial models of oral mucosa.; Diversas patologías afectan sustancialmente la estructura histológica y la fisiología de la mucosa
oral humana haciendo necesario el uso de trasplantes de mucosa oral. En este contexto, el escaso
número de donantes ha incrementado la necesidad de producir sustitutos artificiales de mucosa oral
humana, la mayoría de ellos sin atender a las diferencias histológicas encontradas en las diferentes

Objetivos: Comparar qué factores sanitarios y socio-demográficos presentan los neonatos que
consultan en las urgencias hospitalarias del hospital respecto a un grupo control de neonatos que
no realizan ninguna visita en su primer mes de vida.
Métodos: Se realizó un estudio retrospectivo observacional de casos y controles, considerando como
casos aquellos neonatos que acudieron a urgencias durante su primer mes de vida, emparejados
con controles del mismo sexo y edad nacidos en el hospital pero que no visitaron el servicio de
urgencias.
Resultados: El 15,15% de los recién nacidos en un año visitaron las urgencias. Los factores asociados
con más consultas al servicio de urgencias fueron: Desempleo, trabajo como autónomo, y edad
más joven de la madre. Los motivos de consulta más frecuentes fueron: Trastornos fisiológicos del
neonato (17,4%), infección respiratoria superior (13,4%) y cólico o llanto (12,8%).
Conclusiones: Los neonatos pueden presentar una gran variedad de síntomas y signos que por
su inespecificidad generan gran ansiedad en sus padres y numerosas visitas a los servicios de
urgencias y con alto porcentaje de ingreso, probablemente más por prudencia del médico que por
su gravedad clínica.; Objectives: Compare what sanitary and socio-demographic factors present the newborns that were
attended in the hospital emergency room in regard to the newborns who don’t visit the hospital
during their first month of life.
Methods: A retrospective observational case-control study was performed, considering as cases
those neonates who were brought to the hospital emergency department during their first month of
life, matched with control of their same sex and age, born in the hospital but that hadn’t visited the
emergency room.
Results: 15.15% of the newborns were brought to the hospital emergency department during the
year of the study. The main risk factors associated with the visits to the emergency room were:
Unemployment, self-employment, and the mother’s younger age. The main reasons why the
newborns were brought were: Phisiological phenomena of the newborn (17.4%), upper respiratory
infection (13.4%), and colic or crying (12.8%).
Conclusions: Newborns can manifest a wide variety of symptoms and signs that, due to their nonspecificity, generate anxiety in their parents and can generate numerous visits to the emergency
services, with a higher probability of hospitalization due to their young age and the physician’s
caution, rather than due to severity of the illness

Introducción: Para el diagnóstico de Artritis Reumatoide (AR), se emplean la detección de dos
autoanticuerpos; el factor reumatoide (FR) y los anticuerpos anti-péptidos citrulinados cíclicos (APCC).
La presencia de estos anticuerpos determina que la AR se clasifique como seropositiva o seronegativa
con importantes implicaciones tanto en el desarrollo clínico como en el pronóstico de la enfermedad.
Objetivo: Valorar el papel de los APCC y el FR en el diagnóstico de la AR, así como su influencia en la
instauración del tratamiento farmacológico.
Metodología: Se estudiaron 213 pacientes con sospecha clínica inicial de AR con determinaciones
de FR y APCC. Se estudiaron su diagnóstico definitivo y el tratamiento farmacológico instaurado.
Resultados: 63 de 213 pacientes tuvieron diagnóstico definitivo de AR. El 77,8 % se clasificaron como
seropositivos. La sensibilidad y especificidad fueron superiores en los APCC frente a FR. Inicialmente
el FR presentó un escaso valor diagnóstico ya que resultó negativo en más de dos tercios de los
pacientes con diagnóstico de AR. El ajuste preciso de su valor de referencia permitió su utilización
como una herramienta diagnóstica útil. La presencia de anticuerpos en pacientes con AR influye en el
tratamiento de elección con una mayor utilización de metotrexato frente a otros inmunosupresotres
en los pacientes seropositivos.
Conclusiones: Los APCC presentaron mejores parámetros de validez de prueba diagnóstica que el FR.
La presencia de anticuerpos en la AR influye en el tratamiento de elección posiblemente por la mayor
severidad de la patología.; Introduction: For the diagnosis of Rheumatoid Arthritis (RA), the detection of the autoantibodies,
rheumatoid factor (FR) and cyclic citrullinated peptide antibodies (APCC) are used. The presence of
these antibodies determines that the RA is classified as seropositive or seronegative, with important
implications both in the clinical development and in the prognosis of the disease.
Objective: To assess the role of APCC and RF in the diagnosis of RA, as well as its influence on the
establishment of pharmacological treatment.
Methodology: We studied 213 patients with initial clinical suspicion of RA and with determinations
of FR and APCC. Their definitive diagnosis and the pharmacological treatment established were
studied.
Results: 63 of 213 patients had a definitive diagnosis of RA. 77.8% were classified as seropositive. The
sensitivity and specificity were higher in the APCC compared to FR. Initially the RF presented a low
diagnostic value since it was negative in more than two thirds of the patients with a diagnosis of RA. The
precise adjustment of its reference value allowed its use as a useful diagnostic tool. The presence of
antibodies in patients with RA influences the treatment of choice with an increased use of Methotrexate
against other immunosuppressants in seropositive patients.
Conclusions: The APCC presented better parameters of diagnostic test validity than the RF. The
presence of antibodies in RA influences the treatment of choice possibly due to the greater severity of
the pathology.

Objective: Given the relevance of scientific training in undergraduates whatever their future professional
practice and viewing that consistent bibliographic usually limit scientific training to a component of the
medical curriculum, this approach intends to go a step further seizing each discipline of the medical
curriculum for developing progressively scientific competencies. In this context, a strategy, integrating
scientific and disciplinary competencies (in this case, Histology and Embryology), is presented
Material and Method: The strategy was applied for fifteen years to 2000 ± 500 students per year while
they were studying Histology and Embryology, one of the first-year disciplines. It included planning steps
carried out by Ph.D. professors as well as the implementing and evaluating ones in charge of these
professors and properly trained teachers
Results: Averages of percentages of enrolled students who did not start the course, who dropped out before
being able to take the final disciplinary evaluation, who could take it and who were promoted to the second year
during fifteen years are registered. Likewise, an average of 80% of them satisfied with this strategy and the
positive impact on the corresponding competencies of Biology, another first-year discipline, are also recorded.
Conclusions: This strategy, implemented between 1986 and 2001, proved viable and fruitfully relevant
until a curricular transformation limited its continuation without generating its equivalent replacement.
Beyond the elapsed time since then and the reasons hindering its full accomplishment, this long-standing
approach is rescued so that it may eventually be considered and improved by those who may value it
profitably for their curricula.; Objetivo: Dada la relevancia de la formación científica en los estudiantes de grado, cualquiera sea su futura
práctica profesional, y dado que consistentes reportes bibliográficos limitan usualmente el entrenamiento
científico a una parte del currículo, este enfoque intenta ir un paso más allá aprovechando cada disciplina del
currículo médico para desarrollar progresivamente competencias científicas. En este marco, es presentada
una estrategia que integra tales competencias con las histológico-embriológicas.
Material y Método: Dicha estrategia fue implementada durante quince años a 2000 ± 500 alumnos por año
mientras cursaban Histología y Embriología, una de las disciplinas de primer año, e incluyó etapas planificadoras a cargo de Profesores Doctores, así como otras, implementadoras y evaluadoras, a cargo de éstos y
de docentes debidamente entrenados.
Resultados: Son registrados los promedios de porcentajes de alumnos inscriptos que no iniciaron el curso,
que desertaron antes de estar habilitados para el examen final disciplinar, que lo cumplieron y que se promovieron a segundo año a lo largo de ese periodo. Asimismo, se da cuenta del promedio de satisfacción del
alumnado con esta estrategia (80%) y de su impacto positivo sobre las correspondientes competencias en
Biología, otra disciplina de primer año.
Conclusiones: Esta estrategia, implementada entre 1986 y 2001, se mostró viable y fructuosamente relevante hasta que una transformación curricular limitó su prosecución sin generar su equivalente reemplazo.
Más allá del tiempo transcurrido desde entonces y de las razones que dificultaron su pleno cumplimiento, se
rescata este perdurable enfoque para que eventualmente sea considerado y mejorado por quienes lo valoren
de manera provechosa para sus currículos.
