Hormonas sexuales endógenas y cáncer de próstata

Abstract

Introducción: Analizar el comportamiento hormonal en el cáncer de próstata (CP), así como la relación testosterona-PSA y testosterona-agresividad tumoral, y comparar dichos resultados con los obtenidos en pacientes con Hipertrofia benigna de próstata (HBP). Materiales y métodos: Se ha realizado un estudio observacional de casos y controles, entre Enero y Octubre del 2011. Seleccionamos 30 casos de diagnosticados de CP y 15 controles diagnosticados de HBP. Las concentraciones de testosterona total, SHBG-testosterona, FSH, LH, Prolactina, Progesterona y Estradiol fueron medidas ambos grupos. Para el análisis estadístico calculamos el test de correlación de Pearson y el test-t de Student entre variables de ambos grupos, utilizando el paquete estadístico SPSS 17.0. Resultados: Se observó en los casos una correlación negativa entre PSA y testosterona (p=0,03); positiva del PSA con el estradiol y la progesterona (p<0,01); y positiva entre LH y FSH (p<0,01). No se han encontrado diferencias significativas en los valores hormonales entre grupos ni entre los pacientes con CP según su agresividad, así como correlaciones significativas entre los valores dentro de cada grupo. Conclusiones; Dada la variación de los niveles de testosterona con la edad, serían necesarios estudios que permitan la monitorización de los mismos a largo tiempo, lo cual nos permitiría aclarar el papel real de la testosterona en el CP.

Introduction: To analyze the hormone behavior in the prostate cancer (PC), as well as the relationship between PSA-testosterone and testosterone-aggressiveness tumor and compare these results with those obtained in patients with benign prostatic hypertrophy (BPH). Materials and Methodology: We performed an observational study of cases and controls, between January and October 2011. We selected 30 cases diagnosed with PC and 15 controls diagnosed with BPH. The concentrations of total testosterone, SHBG-testosterone, FSH, LH, Prolactin, Progesterone and Estradiol were measured in both groups. For statistical analysis we calculated the Pearson correlation test and Student t-test between variables in both groups, using the statistical package 17.0 SPPS. Results: In the cases group it was found a negative correlation between PSA and testosterone levels (p = 0.03), positive PSA with estradiol and progesterone (p <0.01), and positive relationship between LH and FSH (p <0.01). No significant differences in hormone levels were found between groups or between patients with PC based on their aggressiveness, as well as significant correlations between the values within each group. Conclusions: Given the variation in testosterone levels with age, studies would be needed to allow the monitoring of these long time, which would allow us to clarify the actual role of testosterone in the CP.
